MyHealth is available upon registration at any Broward Health hospital. At registration, you will have the option to receive an email invitation with instructions on activating your account.

If you are an existing patient of Broward Health and would like to request access to your medical records, please call the Health Information Management Department at the facility where you received treatment. You will also need to submit the following forms to receive access.
- Florida Health Information Exchange (HIE) Consent / Universal Patient Authorization Form
This form allows Broward Health the ability to view the patient's health information from other providers.
- Patient Portal Consent Form
This form allows a patient or representative access to a website to view their patient's health information.
- Personal Representative Designation Form
The Personal Representative Designation form must be completed for all persons 18 and over who are NOT the legal Power of Attorney (POA) and who are NOT the patient.
If you no longer wish to participate in the Florida HIE, or have access to MyHealth, please fill out the Revocation of Authorization Form and submit it to your Health Information Management Department.
If you feel there are changes that should be made to your medical record, submit a Request to Amend Medical Information Form to your Health Information Management Department.
